Case Summary
After diagnosing tooth number 46 with necrotic pulp, I proposed RCT for a 55 year old man. Challenge in this case was to eliminate the chief complaint of the patient which was the sensitivity around the tooth and of course the underlying pain.

CONCLUSION
The chief complain from the beginning of the patient was that sensation around the involved tooth which was occurring before the treatment and after the treatment was completed the same problem was still arising. There was no signs of Tender on Percussion positive ,no signs of any periapical infection, no over or short obturation ,exact working length with proper lateral condensation obturation was done which was confirmed after taking an IOPA for the same tooth. Patient was advised to take anti-inflammatory medication for next three days. After three days patient reported to the clinic but the problem still persisited. All the possible conditions was looked in for the consideration and there was no signs seen radiographically and also clinically. Still the problem persisted so I looked down deep to the surrounding structures of the involved tooth. From buccal muccosa to tongue and palate also I checked thoroughly. There were very small rounded like structure seen on the buccal mucosa. The features were characteriscally for the condition of Burning mouth syndrome. Patient has earlier reported of some kind of itchiness like spicy food also and discomfort in the surrounding area of that tooth specially on lingual side of the tooth. Patient typically presented all features of burning mouth syndrome and was then diagnosed as BURNING MOUTH SYNDROME.
BURNING MOUTH SYNDROME
It is an ongoing or recurrent burning in the mouth with no obvious cause. Often described as burning, painful, scalding or tingling like feeling in the mouth that may occur every day for months or may be even longer. Dry mouth or an altered taste in the mouth may accompany the pain.
Mainly found in adults in mid 40s-50s. It is estimated to be more frequent in women in men.
MANAGEMENT
There is no specific test for this condition which makes it hard to diagnose. No specific treatment works for everyone. However, medication can be prescribed to manage pain, dry mouth or other symptoms .Just avoid acidic foods, spicy foods.Limit hot beverages. Rinse your mouth to neutralize acid and cool the burning sensation.
